A W9 form must be submitted to the Purchasing Office before a vendor number can be assigned.
Please forward all forms to Debbie Lehman for approval.

  • Form must be legible 
  • Form must be signed and dated 
  • Include a brief description of the reason for assigning a vendor number to this company or individual

    (e.g. “Vendor will conduct a full day workshop for all elementary language arts instructors at the TLC on July 26”)

         (e.g. “Vendor will provide lodging for debate students and team sponsors while attending state level competition in Austin, March 6-7”) 

  • Include name of C-FB staff member making this request
